Reports and recommendations
A diagnostic assessment report is more than a document — it is a practical tool that families, schools and professionals can act on.
Every diagnostic assessment I carry out results in a comprehensive written report. The report is written to be understood by parents as well as professionals, and is detailed enough to be used directly by schools, SENCOs and local authorities.
What the report includes
Background and history
A summary of the information gathered before the assessment, including developmental history, school experience and any previous assessments or interventions.
Assessment findings
A full breakdown of standardised test results across all areas assessed — reading accuracy, reading fluency, spelling, phonological processing, working memory and processing speed — with scores presented clearly and explained in plain language.
Diagnostic conclusion
A clear statement of whether the assessment findings meet the criteria for a diagnosis of dyslexia, along with an explanation of the learner's specific profile of strengths and difficulties.
Practical recommendations
Specific, actionable recommendations tailored to the individual — covering classroom adjustments, teaching strategies, home support and, where appropriate, specialist intervention or further referral.
Exam access arrangements
Where the evidence supports it, the report will include a recommendation for access arrangements such as extra time, a reader or a scribe, in line with JCQ requirements.
EHCP and school provision
The report is written in a format that can be used as evidence in Education, Health and Care Plan applications and to support school-based provision reviews.
Format and delivery
Reports are delivered as a PDF document, usually within two weeks of the assessment. I am happy to talk through the findings with you once you have had time to read the report. If your child's school or SENCO has questions, I am also happy to liaise directly with them.
A note for schools
If you are a school or SENCO commissioning an assessment for access arrangement purposes, please note that a separate written report is not produced for access arrangement assessments. In those cases, I complete and sign the JCQ Form 8 directly. A full diagnostic report is only produced as part of a diagnostic assessment.
